In this section of the CRC, you'll find the "back page" of every Clinical Privilege White Paper, which contains a summary of the training, education, and experience necessary for a practitioner to request the privileges in question. (Note: The forms in this section show only a portion of the data found in the Clinical Privilege White Papers. Because they omit the full complement of information typically necessary to make privilege delineation decisions, we recommend that you review the full White Paper to read the important background information, positions of relevant societies and academies, other interested parties, etc.) These summaries of privileging criteria are categorized by specialty, subspecialty, procedure, or AHP area.
